CONSENT DECLARATION

I, the undersigned, acknowledge that Bayo Pay (M) Sdn Bhd (“Bayo”) is:

• An approved issuer of a designated payment instrument under the Financial Services Act 2013 [Act 758]; and
• Recognised as an approved issuer of a designated payment instrument for wage payments under Section 25(4) of the Employment Act 1955 [Act 265].

I hereby give my explicit consent for Bayo’s e-wallet to be used for the purpose of crediting my salary and wages but not limited to such purpose.

I further provide my explicit consent in accordance with the Personal Data Protection Act 2010 for Bayo to share my virtual account (VA) number with my employer to facilitate the payment of salary and wages.

I declare that I have read and understood the above information, and I voluntarily provide my written consent.

In the event of any inconsistency or ambiguity between translations, the English version of this declaration shall prevail.
